Community Participation

Social and Community Participation is a category of support provided under the National Disability Insurance Scheme (NDIS) in Australia. It is designed to assist NDIS participants in engaging with their community, participating in social activities, and developing essential social skills.

The aim is to enhance their overall well-being, independence, and quality of life by fostering meaningful connections with others and being actively involved in community life.

The Social and Community Participation funding aims to enable NDIS participants to be active members of their communities, promoting inclusion, social interaction, and a sense of belonging.

The National Disability Insurance Scheme (NDIS) includes two broad categories of supports in a participant’s plan: CORE supports and CAPACITY BUILDING supports.

Support Coordination

Light Your Path provides support coordination nationally. Our service involves helping NDIS participants navigate the complexities of the scheme, connect with appropriate services, implement their NDIS plans effectively, and resolve any challenges they may encounter.

Support coordinators at Light Your Path are a dedicated and highly qualified group of professionals with extensive expertise in the National Disability Insurance Scheme (NDIS).

With backgrounds in disability support, psychology, social work, occupational therapy, and disability studies, our coordinators comprehensively understand the NDIS framework and its intricacies.

Support Coordinators at Light Your Path take a person-centred approach, prioritising the participant’s unique needs and preferences, and they build meaningful relationships by actively listening to the participant’s goals and aspirations.

Compassion, empathy, and a non-judgmental attitude are fundamental to our work, ensuring participants feel understood and respected.

In-Home Support/Personal Care

A participant’s home should be a safe place of comfort and pride. Light Your Path respects the homes and lifestyles of the people we support. We want to foster and grow what makes our participants special to assist them in achieving their personal and health goals.

Our friendly staff can assist participants in getting their day off to the right start. No matter how much or how little support a participant needs to get going, we have motivated support workers to make getting up and going out easier. Our staff provide daily services, including safe and informed hoist assistance, getting dressed, personal grooming, laundry and dishwashing, showering and toileting, and brushing teeth.

Our trained professional staff follow the six R’s (Right Medication, Right Person, Right Dose, Right Time, Right Route, Right Documentation) to administer medication as per Dr/specialist’s written instructions.

Light Your Path is excited to achieve new goals with our participants. We help our participants plan meals, grocery shop, cook and clean, exercise and develop good hygiene habits to build skills to live a healthy and rewarding lifestyle.

Development Of Life Skills

Development Of Life Skills

We acknowledge that a person’s skill set is crucial to their sense of identity. Hence, at Light Your Path, we prioritise fostering existing skills and developing new skills to allow our participants to enjoy the opportunities available to them.

Light Your Path will work with participants to best utilise their skills to accomplish meaningful goals and health outcomes. We will help develop new skills to enhance our participant’s quality of life by opening opportunities for them to become more independent and contribute to family life at home. Improving skills creates a sense of pride and belonging in their communities.

We enhance existing skills by motivating participants to design routines that allow their particular skillset to be used regularly. New skills are introduced to accommodate participants’ long-term goals and wants/needs. Light Your Path uses a social support model in which the support worker facilitates participants to experiment and improve skills via participation in all aspects of life.
